Try EverWell is committed to providing transparent, ethical, and patient-centered care. As a patient accessing clinical services through the Try EverWell platform, you have the following rights.
You Have the Right To:
Accurate information
Receive honest, clear information about any treatment being considered, including its potential benefits, risks, limitations, and alternatives.
Informed consent
Make voluntary, informed decisions about your care before any treatment begins. You may ask questions and expect understandable answers.
Know what you’re receiving
Understand whether a medication is FDA-approved or compounded, and what that distinction means for your care.
Transparent pricing
Receive a clear, itemized disclosure of the cost of any program, product, or service before committing.
Know the duration of your program
Understand the expected or estimated length of any treatment plan.
Clinician qualifications
Request information about the qualifications and licensure of the clinician who reviews and manages your care.
Refuse treatment
Decline any treatment at any time without penalty.
Privacy and confidentiality
Have your protected health information collected, stored, and used in accordance with HIPAA and applicable state law.
Ask questions without pressure
Inquire about the health risks, nutritional content, and clinical basis of any weight management or wellness program offered through Try Everwell.
Non-discrimination
Receive respectful, equitable care regardless of race, ethnicity, gender, sexual orientation, disability, or socioeconomic status.
Weight Management Program Disclosures
In accordance with applicable state consumer protection laws, including the Florida Weight-Loss Consumer Bill of Rights (FL Statute 501.0575) and similar statutes in other states, Try EverWell discloses:
WARNING
Rapid weight loss may cause serious health problems. Rapid weight loss is weight loss of more than 1.5 to 2 pounds per week, or more than 1% of body weight per week after the second week of participation in a weight-loss program.
Consult your personal physician before starting any weight-loss program.
Only permanent lifestyle changes such as making healthful food choices and increasing physical activity promote long-term weight loss.

Additional Information
This Bill of Rights applies to all patients accessing clinical services through the Try EverWell platform.
Try EverWell is a technology coordination platform and does not employ clinicians. Clinical care is provided by independent, licensed healthcare providers.
Nothing in this Bill of Rights creates a provider-patient relationship between Try EverWell and any patient.
